Office Payment Policies

Thank you for choosing us as your primary care provider.  We are committed to providing you with quality and affordable health care.  Because some of our patients have had questions regarding patient and insurance responsibility for services rendered, we have developed a payment policy.  All patients will be asked to review and sign the complete policy at your first visit.

1. Insurance: We participate in most insurance plans, including Medicare.  If you are not insured by a plan we do business with, or do not have an up-to-date insurance card,  payment in full is expected at each visit.

2. Co-payments: All co-payments and deductibles must be paid at the time of service.  You will be asked to re-schedule your appointment if you are not able to provide co-payment.

3. Claims: We will submit your claims and assist you in any way we reasonably can to help get your claims paid.  Please be aware that the balance of the claim is your responsibility.  If your insurance company does not pay your claim in 45 days, the balance will automatically be billed to you.

4. Additional Fees: Our office may charge additional fees for services performed by a provider without a patient visit.  These include a $15 fee for prescription refills,  a $10-30 fee (depending on length) for forms (camps, college entry, disability, employment, insurance, adoption).  These fees are generally not reimbursed by insurance.  You are always welcome to schedule an appointment to address these needs if you prefer.
